I think in Crystal it became the clear bell

That's somewhat true...but not entirely.
Teru-sama is what the game refers to for something that's not there. It is basically the Missingno. of items, in which you have an item with a Hex that is not used, which is the same basic thing that happens with a Missingno. encounter in RB...

Teru-Sama is just what the game places as the name of the item that does not exist, so in saying that it was 'replaced by the Clear Bell' you're not truly correct. A SINGLE HEX was changed to refer to the CLear Bell, but there are plenty more that are unused, and these would also show 'teru-sama' up in your tiem list.
